Chemical Spill Trajectory Model Request Proforma

This document is for use by National Plan stakeholders to obtain predictions from the Chemical Spill Trajectory Model for use during an incident, exercise or for contingency planning purposes. The proforma is not for public use. To access the form please read the DISCLAIMER below and follow the link.

The Request can be filled out on-line or printed, completed and faxed to 02 6279 5076 for immediate attention. After hours fax requests to be sent to 02 6230 6868.  

After sending the request, receipt should be confirmed by telephoning Rescue Coordination Centre, Australia (Emergency Response Division) on 02 6230 6811, or 1800 641 792.

Please note that one month's notice is required for CSTM runs requested as part of an exercise or for contingency planning purposes.
