Application Forms
Below is an index of Forms required for updating or applying for a Marine Qualification. To access a form, simply click on the desired title and follow the instructions given.
Applicants are referred to the Guidance Notes regarding AMSA requirements that are contained within form AMSA 63 or AMSA 107.
Applicants are reminded when applying for an assessment of eligibility for issue of an Australian certificate of competency, applicants must submit original letter(s) from the shipping company, written on company letterhead paper, verifying the qualifying sea service (Marine Orders Part 3, provision 7.2).
The normal time to process an application is 20 working days. If the application form is missing vital information, incorrectly completed or the required supporting documentation has not been submitted, the application will be returned to the applicant to be completed correctly. The application will then need to be re-submitted, this will cause delays in the issue of a Certificate.
