Information Required

Incident Alert - An Incident Alert form is to be completed and this information is to be forwarded from the operator to the Inspectorate as soon as practicable and within 4 hours of the incident being made known to the operator.

Incident Report - An incident report is a more detailed follow up report and must be forwarded from the operator to the Inspectorate within 72 hours.
