We have listened to your feedback and made some changes to simplify and improve a number of our national system forms.
Two updated forms are now available:
New AMSA 504
There is now one single form to apply for, vary or renew a certificate of operation.
The old form AMSA549-Application to vary a certificate of operation is no longer available.
New AMSA 521
There is now one single form to apply for, or renew a certificate survey and/or load line.
The old form AMSA576-Application for a renewal of a certificate of survey and/or load line is no longer available.
Other improvements
Both of the new combined forms have a number of other improvements including:
- We have removed a number of previously mandatory fields throughout the form.
- We have added automated look-up fields for addresses.
- ASIC and ABN documentation is now no longer mandatory.
- 'help' commentary has been included for many fields.
We hope to roll out more practical changes over the coming months.
