We are launching the first ever safety lessons learnt as the result of a marine incident investigation report. Every month we will be sharing an overview of a domestic commercial vessel incident investigation and the safety lesson you can learn to prevent a similar incident occurring on board your vessel. 

This month's report is a serious injury to a pilot on a vessel during bad weather. The pilot was seriously injured when a large wave impacted the vessel while they were standing in the cabin.
