This is evidenced by significant differences between oil record book entries and daily sounding records and the fact there was insufficient capacity in the ship's oily water retention facilities. |
Other serious deficiencies: | Numerous instances of crew's minimum resting hour in any 24 hour period was under 10 hours. One crew had one case of 4 hours rest in a 24 hour period. The three operational generators—four in total, one under maintenance—were unable to synchronize and share the load. The vessel's oily water retention capacity not able to deal with the oily water generated by vessel's machinery. |

The ships safety management system fails to make sure procedures and instructions for key shipboard operation and safety of navigation is effectively implemented. |

Master has a repeated breach in not complying to safety of navigation, vessel safe routeing in adverse weather conditions and protection of environment. 60 containers from Bay 52 and 21 containers from bay 56 lost overboard exceeded tier stack limits set in Cargo Securing Manual. 276 containers from bay 01 to bay 60 intact in stow exceeded tier stack limit set in cargo securing manual. |
