Load line survey report

Overview

Surveyors – use this form to report on a domestic commercial vessel's compliance with conditions of assignment for a load line. Complete this report at the vessel's initial and periodic surveys. 

Submitting the form

Submit the completed form to the MARS system with the relevant survey code and keep a copy for your records.
