Notification of a change to accreditation

Overview

You can use this form to notify us of a change to your accreditation including:

change in place of employment
change of address and/or contact details
change of name or any other change that may result in confusion as to your identity
membership of a professional association revoked, suspended or involuntarily cancelled.

Submitting the form

Due to the current uncertainties with the outbreak of COVID-19 (Coronavirus), to avoid applications being delayed, we ask that you DO NOT post your application to AMSA.
