Survey activity report and temporary operations

Overview

Surveyors – use this form to:

• make a survey activity report
• document non-conformances
• permit temporary operations under Exemption 07
• communicate these matters to us and the vessel owner.

Submitting the form

When the form is completed:

• Provide a copy of the completed form to the owner of the vessel. The owner must keep this form on the vessel when a related exemption is in force.
• Submit the completed form to the MARS system with the relevant survey code.
• Keep a copy for your records.
