The Safety Management System has failed to ensure the vessel is properly maintained as evidenced by starboard side accommodation ladder found with cracked welds between the side rails and treads, quantity of portable lashing equipment provided on-board not as per cargo securing manual. Inventory records indicate short falls since prior December 2018, diesel generator turbo charger number 1 missing some cover plates, exposed hot surfaces in excess of 220oC and steering gear reserve hydraulic oil storage tank below minimum level for refill of one system
