Safety Management System does not ensure critical shipboard operations are carried out effectively per ISM Code 7 as evidenced by crew not following company procedures for ECDIS safety depths (a safety depth was used that was less than the vessel draft) and unfamiliar with ECDIS look ahead functions; cargo not trimmed per grain code stability calculations.
Safety Management System fails to ensure compliance with ISM Code 6 relating to resources and personnel as evidenced by crew not adequately rested, reports of non-conformities not properly managed and captured in audits.
Safety Management System is not effective in ensuring Element 7, 8 & 10 of the ISM Code are met as evidenced by the use of inappropriate charts, lack of manual position fixing, lack of maintenance of emergency equipment, handrail gates being seized, inadequate insulation to No. 2 and 3 DG Turbocharger exhaust gas ducting and Fuels leaks from filter and pump.
