Serious deficiencies relating to ISMThe Safety Management System failed to ensure the vessel is prepared for an emergency as evidenced by crew unfamiliar with rescue boat retrieval system, rescue boat davit instructions are incomplete, port inboard life raft secured by two independent release units, crew unfamiliar with bridge emergency steering changeover procedures, engine room bilge emergency bilge suction valve not able to be operated and there are no ships specific instructions for the engine room water mist firefighting system.
The Safety Management System also failed to ensure the vessel has been adequately maintained as evidenced by the defective rescue boat, emergency battery system earth fault meter disconnected,  emergency battery system has a dead-short, fuel oil and ballast tank vent heads defective, main deck fuel oil sounding pipes corroded, port side bosun’s store door not watertight, diesel generator number 1 fuel oil leak non-driving end, diesel generator number 1 fuel oil filter cover leak, diesel generator number 1, 2 and 3 shaft seal leak driving end and four engine room tank sounding pipe cocks do not close.
Other serious deficienciesRescue boat defective. Aftermarket parts of an alternate design where sourced. The rescue boat’s original manufacturer was contacted and agreed that they would be a suitable alternative (letter provided); Class approved the changes. As a side note, based on my experience I also could see no issues with the proposed modification. The vessel reported it would source the originals in any case as they are still available from the manufacture (but with a little delay).

Serious deficiencies relating to ISMThe Safety Management System failed to ensure effective maintenance of ship and its equipment, as evidenced by port navigation light not complying with COLREGs, securing bracket for light/smoke signal on port bridge wing  wasted and secured with cable ties, free fall lifeboat release hook alignment marks not visible. Unable to ascertain whether hook has been reset correctly, rescue boat outboard motor not SOLAS compliant and missing safety guard, insufficient fuel in fuel tank for rescue boat, earth lamps on battery charging panel inoperative, numerous electrical boxes on deck wasted, ballast and fuel tank air vent heads not  maintained. Securing bolts missing, cover plates wasted, temporary repairs carried out, sealing rings dislodged, bosun store bilge well (Port side) suction obstructed with old rags, main engine rpm indicator on bridge wing (Starboard) displaying incorrect revolutions, water level ingress system indicating intermittent sensor failure alarms, ECDIS settings not appropriate for vessels area of operation. Safety contour/safety depth/cross track settings incorrect, chart permits not available (two ENCs) for next voyage, EGC not configured to receive AUSCOAST marine safety information. Crew unaware of current AUSCOAST warnings for vessels current area of operation, large amount of garbage stored in forward bosun’s store, crew unfamiliar with operation of fire dampers located on each deck in accommodation for air conditioning return duct, vessel performed previous voyage from Lumut, Malaysia to Newcastle, Australia with unsecured hatch covers. No evidence of hatch cover cleats used, cargo not removed from drainage channel, water drain lines blocked, a number of alarms (some critical) have been put off scan. Vessel operating in UMS mode and engine room access not secured.
