Serious deficiencies relating to ISM

The Safety Management System failed to ensure effective maintenance of ship and its equipment, as evidenced by port navigation light not complying with COLREGs, securing bracket for light/smoke signal on port bridge wing wasted and secured with cable ties, free fall lifeboat release hook alignment marks not visible. Unable to ascertain whether hook has been reset correctly, rescue boat outboard motor not SOLAS compliant and missing safety guard, insufficient fuel in fuel tank for rescue boat, earth lamps on battery charging panel inoperative, numerous electrical boxes on deck wasted, ballast and fuel tank air vent heads not maintained. Securing bolts missing, cover plates wasted, temporary repairs carried out, sealing rings dislodged, bosun store bilge well (Port side) suction obstructed with old rags, main engine rpm indicator on bridge wing (Starboard) displaying incorrect revolutions, water level ingress system indicating intermittent sensor failure alarms, ECDIS settings not appropriate for vessels area of operation. Safety contour/safety depth/cross track settings incorrect, chart permits not available (two ENCs) for next voyage, EGC not configured to receive AUSCOAST marine safety information. Crew unaware of current AUSCOAST warnings for vessels current area of operation, large amount of garbage stored in forward bosun’s store, crew unfamiliar with operation of fire dampers located on each deck in accommodation for air conditioning return duct, vessel performed previous voyage from Lumut, Malaysia to Newcastle, Australia with unsecured hatch covers. No evidence of hatch cover cleats used, cargo not removed from drainage channel, water drain lines blocked, a number of alarms (some critical) have been put off scan. Vessel operating in UMS mode and engine room access not secured.
