The safety management system, as implemented, fails to ensure the crew are adequately rested as evidenced by systematic and serious rest hour breaches for crew in June, July and August 2019, the vessel is maintained to relevant rule and regulations as evidenced by port lifeboat grab line defective and one emergency light on aft mooring station defective and the ship board operations are carried out effectively as evidenced by lashing bar securing nuts not in place. The vessel cannot respond to an emergency—risk assessment attached to flag dispensation 90923-278795 not being followed
The Safety Management System, as implemented did not ensure the vessel is prepared for an emergency as evidenced by access blocked to the dewatering valves in the forward boatswains store and main engine lube oil tank port side found with sight glass self closing valved gagged open. The vessel has also not been effectively maintained as evidenced by 440V main switchboard showing earth fault and incomplete exhaust lagging on main engine turbo chargers 1, 2 and 3 and diesel generators 2, 3 and 4. Hot surfaces in excess of 220oC
