Deficiencies are evidence that the vessel has not been maintained between surveys as Safety of Life at Sea convention states (SOLAS Chapter 1 Regulation 11). The safety management system fails to ensure reports of non-conformance notes do not reoccur as evidenced by emergency generator fault not reported to Flag, Class or Port State and repeat of deficiencies.

Other serious deficiencies:

Seafarers not being paid their full wages each month as no evidence could be presented on board that allotment of wages are being paid regularly and each month.
Emergency generator not auto starting or connecting to emergency switchboard automatically. Both starting batteries are unserviceable.
Freefall lifeboat - second independent rechargeable energy source of starting failed. (Number 1 battery unserviceable).
