Crew members have not monitored the vessels position on nautical charts during the voyage. Last plotted position on charts was 29 hours before vessel berthed. Numerous charts have no positions plotted including all pilotage charts. Crew have used unapproved ECS for navigation paper charts onboard with no positions during harbour navigation [10]; photo. A evidenced by the failure to ensure cargo is secured, defective ER alarm control computer, failure of the digital controller for ME speed, defective self closing cocks in the ER, numerous bilge alarms off scan, defective paint locker sprinkler, defective pilot ladder and uncorrected nautical charts numerous electrical connections found in focsle unsecured [20]; crew unaware of status of alarms due to system being turned off [21]; [22]remote valve for No2 HFO settling tank did not work even when bolt removed [22]; several sounding pipes in ER found with self closing deVictoriae defective [23]; water ballast tank air vent blocked with chock piece from pilot ladder [24]; dirty mirrors on mag compass [25]; charts from last completed voyage not corrected [26]; pilot ladder defective [27]; pilot ladder defective - splices added to side ropes [28]; galley self closing door held open [29]
Other serious deficiencies Repeated instances of crew not being paid as per SEA. Some instances of rest hour records
not reflecting actual hours worked. Three instances in February of crew members not being fit for duty. 2 SCBA in focsle defective. Starboard engine room fire damper defective. Sea load is 600kW and only 2 functional generators with a max capacity of 575 kW each. Several remote closing valves for HFO tanks tanks defective.

The safety management system as implemented onboard does not make sure that procedures are implemented effectively for key shipboard operations as evidenced by:

  • failure to fix positions on the ECDIS
  • officers being unable to demonstrate MF/HF on reserve power
  • officers unable to demonstrate the operation of rescue boat launching on reserve power
  • excessive oil accumulation in the engine room
  • engine room work and rest hours not reflecting reality
  • serious deficiencies mentioned below
  • excessive oil accumulation on and under main engine.

Cracks found on six out of eight temporary welds on side bulkhead between decks 4 and deck 5. Photos:

Welding carried out by ships crew. Cracks length range from 300mm-600mm long. Condition of vessel not maintained after survey as per SOLAS Chapter 1, Regulation 11, Flag and RO not notified of defects on bulkhead cracks evident1; cracks2; cracks3; cracks4
