This is evidenced by: erroneous crew working arrangement and records of crew work hours; no records of monitoring of cargo operations; main engine defective and providing insufficient power; generator no.3 defective with no spare parts; boiler gauge leaking steam; and boiler automatic operation failed alarm activated.
