Serious deficiencies relating to ISM The Safety Management System fails to adequately satisfy the requirements of the ISM Code related to shipboard operations, emergency preparedness and maintenance of ship and equipment, as evidenced by out of date electronic charts, lack of familiarity with ECDIS, lack of familiarity with shipboard equipment, defective or inadequate fire prevention arrangements and recorded hours of hours of rest did not reflect actual hours worked by deck and engine officers. 
Other serious deficiencies Fire main isolation valve defective. Engine room dampers 2,3 and 4 defective.

Serious deficiencies relating to ISM The Safety Management System fails to  satisfy the requirements of the ISM Code related to shipboard operations, emergency preparedness and maintenance of ship and equipment, as evidenced by sewage treatment plant defective, accommodation ladders unsafe,  S-band radar not properly maintained, ballast tank air vents not  maintained, hatch cover operating motor leaking oil,  self-closing mechanism and cover for aft peak ballast tank removed and sounding head fitted with a ball valve, cooling water and oil leaks on main engine and auxiliary engines and insulation wetted through oil.
Other serious deficiencies

Sewage treatment plant defective. No evidence of sludge return through visual indicator pipes. Aeration compartment empty.

  • No sludge return visible through indicator pipe as required by maker's instructions (JPG 850 KB) [1]
  • Hatchcover operating motor for #2 (S)  cargo hold leaking oil from shaft seal. (JPG 885 KB) [2]
  • Not totally slack and no split puin fitted to bolt (JPG 893 KB) [3]
