How to report a marine incident relating to a domestic commercial vessel.

If you are a master or owner of a domestic commercial vessel which has been involved in a marine incident, you must report it. 

1. Submit an incident alert

As an owner or master of a domestic commercial vessel, you must make an incident report if your vessel is involved in a marine incident that involves:

  • the death of a person
  • serious injury to a person
  • the loss of a vessel
  • the loss of a person from a vessel
  • significant damage to a vessel.

As soon as reasonably practicable after becoming aware of the incident, you must report the incident to your local marine safety agency.  

This alert can be provided by any means such as phone, email, in person, or using incident report form 529.

2. Submit an incident report

As an owner or master of a domestic commercial vessel, you must report all marine incidents within 72 hours using incident report form 529.
