Overview
You can use this form to:
- apply for a new certificate of survey, load line certificate, or both
- renew an existing certificate of survey, load line certificate, or both
- apply to alter or modify the vessel’s operations
- apply to alter or modify the vessel’s structure, arrangements, material or scantlings
- request changes to an existing certificate, including:
- removing or changing a condition
- changing service categories or capacity
- updating engine details
- changing the vessel name
- updating the current certificate holder’s details (for example, a change of name).
You can’t change the certificate holder using this form. The new holder must complete a separate Application for change of certificate (or approval) holder for a vessel permission (form 1780).
We can’t assess your application until we receive the vessel survey reports.
You’ll receive survey codes after you apply. Give them to your surveyor and ask them to submit their reports as soon as possible.

You can complete and submit the form online or you can print out the form to complete offline.
Please attach a separate document if you need to provide additional information.
Trust arrangements
If you are making an application in the name of a trust, please supply a copy of the trust deed in the supporting documents section of the application. We require the deed to confirm the full and correct name of the legal entity that is making the application. If a copy of the trust deed is not submitted at the time of making the application, it may result in delays processing your application.
Commercial fishers
If you may need to carry a fishery observer on board your vessel, you must apply to carry special personnel in the 'Service category' section of the application. This applies to both certificate renewals and new applications. Find out more about carrying a fishery observer.

- Load line certificates
- If the application is incomplete, it will cause processing delays and may not be progressed. Refer to our incomplete application policy for Domestic Commercial Vessel applications.
