Incident alert (form 18)

Notify us of the marine incident as soon as you can:

• complete the online form below, or
• download and complete this form (PDF, 908KB) and email to reports@amsa.gov.au.

The form must be completed by the owner or master as soon as reasonably practical after becoming aware of the marine incident. For regulated Australian vessels and foreign vessels, submit the form within 4 hours.

Please upload files related to the incident. These could be photos, investigation reports or other media.

Note: Do not include personal information or images, such as passport or driver licence details, or photos of injured people.
