Notification of a change to accreditation

Overview 

You can use this form to notify us of a change to your accreditation including:

  • change in place of employment
  • change of address and/or contact details
  • change of name or any other change that may result in confusion as to your identity
  • membership of a professional association revoked, suspended or involuntarily cancelled.

You can fill in this form online before downloading it as PDF document, or print a blank form.

Submitting the form 

Due to the current uncertainties with the outbreak of COVID-19 (Coronavirus), to avoid applications being delayed, we ask that you DO NOT post your application to AMSA.
