Sewage treatment plant not operational evidenced by dry and full of solidified sludge, no areation or effluent feed observed. No evidence of system in use.
Rescue boat engine not operational. Engine started by battery but no cooling water discharged, engine was over revving and abnormal sounds heard.
Engine room ventilation fan fire dampers 1-4 unable to be closed and sewage treatment plant defective.
Safety Management System fails to adequately satisfy element 8 of the ISM Code related to emergency preparedness as evidenced by defective lifeboats release mechanism; crew unfamiliar with emergency steering; stanchion and lifelines for crew access to bow not available and forward boarding ladder securing shackles seized. Safety management system fails to ensure element 10 of the ISM code in relation to ship equipment, as evidenced by numerous oil leaks; oil soaked insulation; oily rags and waste oil stored in open containers; defective hatch cover closing arrangements, drains, seals and hatch coaming corroded; defective pilot ladders and speed log.
