Initial survey fire safety report
Surveyors can use this form to report on the fire safety of a domestic commercial vessel at initial construction or alteration.
A surveyor who does not have a Quality Management System compliant with ISO 9001 or an equivalent management system as described in National Law - Marine Surveyors Accreditation Guidance Manual 2014 Part 2 Clause 2.9 must use either:
- this form or
- MARS and supporting documentation containing at least the information required in this form.
A surveyor who does have such a Quality Management System may use this form.
Submitting the form
The completed form must be retained by the surveyor for their records and a copy must be submitted to us.
