National plan spill trajectory modelling request (form 95)

Overview

You can use these forms to request a National Plan spill trajectory model for an oil or chemical spill. 

Oil spills: Complete the Oil Spill Trajectory Modelling form (OSTM).

Chemical spills: Complete the Chemical Spill Trajectory Modelling form (CSTM).

Submitting the form

Save the completed form or scan the hand-written document and contact the AMSA Marine Pollution Duty Officer through the AMSA Joint Rescue Coordination Centre (24/7) on 1800 641 792 for further instructions.
