Overview
To be used when transporting livestock as cargo, this form accompanies the application for an Australian certificate for the carriage of livestock (ACCL).
How to complete this form
The Record of equipment and arrangements is an attachment to the ACCL. This is to be prepared by the ship owner or operator and submitted to AMSA for consideration.
The form is for any applicant of an ACCL to complete and provide with their application. It will not be able to be provided initially, but will have to be provided before any ACCL can be issued.
