Response to the Pacific Adventurer incident—strategic issues report, operational and technical issues report

Following the Pacific Adventurer off Brisbane on 11 March 2009, a number of inquiries were undertaken to investigate the circumstances contributing to the cause of, and the response to, the incident.

The Incident Analysis Team has identified a number of issues that were raised either during or after the response. Each issue has been examined in detail using a range of available information sources to ascertain its veracity. Based on this examination a series of conclusions and recommendations are presented.
