Overview
Use this form if you are applying for a Coxswain Grade 3 using a recreational boat licence. If you aren’t sure whether you need this form, check the pathways to becoming a Coxswain Grade 3.
You will need to complete specific on water tasks witnessed by an observer. Make sure the observer meets the requirements for the duties you want to qualify for.

- Give the form to an observer who meets the requirements.
- Complete the practical on water tasks witnessed by the observer.
- If appropriate, the observer signs the declaration on the form.
- You sign the declaration on the form.
Observer requirements
To qualify for all Coxswain Grade 3 duties, your observer must hold one of the following:
- Coxswain Grade 2 or 1
- Master <24m, <45m, or <100m
If you want to work in a marina or mooring area only your observer must:
- be eligible for a Coxswain Grade 3
- have held a recreational boating licence for at least two years
- have at least six months experience operating similar kinds of vessels in a marina or mooring area.
Note: If you are looking to operate as a Coxswain grade 3 limited to marina/mooring area operations, AMSA cannot issue you with a physical Coxswain grade 3 NC card.

Vessel requirements
You will need to complete the practical on water tasks on a vessel that meets the following requirements:

Submit a copy of this form with your Coxswain Grade 3 application.
To work without the certificate or work only in a marina or mooring area, you do not need to submit the form. Keep a copy of this form on board along with your other evidence.
