Submit an incident report (AMSA Form 19)
All commercial vessels involved in a marine incident in Australian waters must submit AMSA Form 19 within 72 hours of becoming aware of the incident.
There are two ways you can report a marine incident. You can use either method to notify us.

Recreational vessels should report incidents to the relevant state or territory maritime safety agency.

Additional personal information, for example, passport details are not required.
Giving false or misleading information is a serious offence under section 137.1 of the Criminal Code. Aiding, abetting, counselling or procuring a person to submit false or misleading information is also an offence.
