This form is no longer in use.
To change the certificate holder for a vessel permission, the new owner or operator should use the form that matches the vessel's permission or approval:
- Certificate of survey and/or load line certificate (form 521)
- Class C restricted operations approval (EX40) (form 523)
- Non-survey vessel approval (EX02) (form 579)
