AMSA Form 1882 replaces AMSA Form 536
If you are applying for the Coxswain Grade 3 through Schedule 3 (option d), the recreational boating pathway, you will need to complete a Coxswain Grade 3 Declaration Form (AMSA form 1882).
To complete the declaration form you will need to be assessed by an observer. See Completing the declaration required for Coxswain Grade 3 for recreational boating licence holders for details on how to complete this form.
Submitting the form
Submit this form with your Certificate of competency application form 426 and other supporting documents at an AMSA contracted Australia Post outlet.
