Surveyors and licensed electrical contractors - use this form to report on low voltage electrical systems on domestic commercial vessels at initial survey.
Submitting the form
Submit the completed form to the MARS system with the relevant survey code and keep a copy for your records.
If the report has been completed by a licensed electrician, email the completed form to DCVApplications@amsa.gov.au.

Use this form to document National Standard for Commercial Vessels Part C 5B, 2.21.2 commissioning test results.
Owners must keep this report with the vessel’s documentation for the life of the vessel.
