  • Unheeded weather warnings result in fishing charter vessel flooding
    A charter fishing vessel proceeding to a fishing spot began experiencing progressively worsening weather conditions and water ingress. Despite the owner’s warning prior to departure and passenger complaints, the master continued to the fishing destination. Strong winds and waves hampered anchoring, with water accumulating on deck through the transom door to the point of flooding. The vessel’s EPIRB was deployed, and a rescue helicopter attended, winching the master and passengers to safety.

  • Passenger vessel collision with unseen submerged rock
    A passenger vessel collided with an unseen rock, resulting in the injury of a passenger. The master navigated the vessel into unfamiliar waters that was not a normal or planned part of the passenger tour experience, and not authorised by the tour company. Owners and operators should ensure masters continue to conform to the planned or authorised route for the services they offer.

  • Planned maintenance failure leads to vessel sinking
    In the early afternoon of 30 May 2022, a hire and drive sailing catamaran began to take on water before sinking shortly after departing the marina. Onboard was the charterer, their family and a briefer who was in control of the vessel. The investigation established that following recent maintenance, an inspection plate had not been replaced, allowing water ingress. It was further identified that the charter company’s maintenance procedures did not suitably document handing over and/or tagging of removed items during maintenance work.

    On Sunday 9 January 2012, the cargo vessel the MV Tycoon broke free from its mooring at Flying Fish Cove, Christmas Island, and washed against the seawall and nearby cliff face. The ship had been loading phosphate at the time of the incident.
